Screen time incorrect

The app icons don’t match the app name and the screen time is counting across all apps despite none of them being open. For example the screen time shows 3 minutes in each app and 18 mins total, but none of them have been opened at all. Being on my phone for one minute adds one minute for every app. All apps are also blocked despite not hitting the blocking limit.
